Terms of Service
Welcome to Squad Health, an online prescription management platform (the “Services”) made available by Squad Health, Inc. (“Squad,” “we,” “us” or “our”). You can access the Services through our web application, text messaging and email communications. Please read these Terms of Service (the “Terms”) carefully because they serve as an agreement between you and Squad. If you do not agree to these Terms or are younger than eighteen (18) years old, do not use the Services.
By accessing or using the Services in any way, including without limitation, browsing the Services, using any information, or submitting any content or personal information via the Services, you agree to and are bound by the terms, conditions, policies, and notices contained in these Terms, including conducting this transaction electronically, certain third-party terms and conditions, disclaimers of warranties, damage and remedy exclusions and limitations, binding arbitration, venue selection, and a choice of California law. You represent and warrant that you are at least eighteen (18) years old and possess the legal right to agree to these terms.
Squad enables you to access one (1) or more of your prescription medication needs (“Order“) and acts as a prescription management platform only. Each medication that you order through Squad must be based on a valid prescription issued for a legitimate medical purpose by a medical professional acting in the usual course of his/her professional practice. The Services are not intended to be a substitute for professional medical advice, diagnosis or treatment, and do not constitute medical or other professional advice. Squad does not recommend or endorse any specific tests, physicians, products, procedures, opinions, or other information that may be mentioned on the Services or by any other electronic means. Reliance on any information provided by Squad is solely at your own risk. The information provided through Squad’s Services is designed to support, not replace, the relationship that exists between patient/site-visitor and his/her physician. Never disregard professional medical advice or delay in seeking it because of something you have read or seen on our Services.
Squad may facilitate the transfer of your prescription to a dispensing pharmacy. By using Squad’s services, you authorize Squad to facilitate the transfer of your prescription to a pharmacy that may offer you the best possible quality of service.

SMS Terms & Conditions.
Opt-in to receive SMS messages can be done by sharing your phone number with your healthcare provider, or verbally, during a conversation with your healthcare provider. You can cancel the SMS service at any time by clicking through the opt-out function of the message. Information (Phone Numbers) will not be shared with third parties for marketing purposes. Messages may be received related to the following:
Message frequency may vary depending on the type of communication. Up to 3 SMS messages per week may be received related to prescription status. Standard message and data rates may apply, depending on the carrier's pricing plan. These fees may vary if the message is sent domestically or internationally.
Mobile Security
Most communication with Squad occurs through your mobile device. Therefore your mobile device likely contains Protected Health Information (“PHI”), such as pictures of your prescriptions, medical insurance cards, and other vulnerable information. As a result, it is of utmost importance that you keep your mobile devices secure at all times. If you are using public Wi-Fi or email applications on mobile devices, you are likely using unsecure mobile networks, putting PHI at risk of interception.
The best ways to protect mobile devices from security breaches is to maintain password protection, encryption, and/or to install a remote wiping/disabling program into them. A remote wiping/disabling program allows users to quickly clear and disable a lost or stolen mobile device, which can possibly prevent or reduce the magnitude of the breach.
While Squad is HIPAA compliant and takes the utmost care in protecting your confidential information, it cannot protect information stored in your mobile device. By using Squad, you agree to hold Squad harmless of any data breaches occurring from theft/misplacement or any third-party intervention with your mobile device.
